I need a programmer to create some web pages based on the following specs.
Statement of Work

1. Create a webpage on your server to capture the following fields
a. Hotel ID 6 characters
b. Rooms numeric, 4 integers, 0 decimals
c. Rate numeric, 4 integers, 2 decimals
d. Date/time provided by the program EG 01/28/2012 14:30
e. UserID 4 characters
f. Shift 1 character
2. Create a webpage on your server to capture the hotel information. Here are the fields:
a. Hotel ID 6 characters
b. Hotel Name 50 characters
c. Hotel Street 50 characters
d. Hotel City 50 characters
e. Hotel State 2 characters
f. Hotel zip 5 characters
g. Hotel phone 10 characters
3. Create a webpage on your server to capture user information
a. Hotel ID 6 characters
b. User ID 4 characters
c. User Name 30 characters
4. Create an administrative webpage that had a mechanism to capture the information for any of the above tables and download the information into a *.CSV file on the user's computer.

Notes:

You will create three SQL tables one for each of the three tasks.
No security whatsoever.
Plain white background. No graphics. Provide a way for me to edit the HTML to personalize the webpages.
Host the webpages for about six months.
